JUNIOR GOLF LEAGUE REGISTRATION FORM
2010 Fall SESSION  Starts SEPTEMBER 11th, 2010.

LEAGUE FEES: for each 12 week session $25.00(non refundable).
Each play day will be $8.00 (Includes tax) for green fees
LEAGUE PLAY DAYS: are each Saturday STARTING AT 10 AM for 12 weeks. We will play a 9 hole round of golf at The North Texas Golf Center Par 3 at 2101 Walnut Hill Lane, Dallas, Texas 75229. All players are expected to participate each week for play. If the junior can not play he/she needs to call The North Texas Golf Center at 972-247-4653 by Friday 12:00 noon prior to play.
ELIGIBILITY: Open to all junior golfers ages 7-17 (who meet the League's qualifications). The junior must not have turned 18 before the last league date in each session. PRIZES: will be awarded by a point system using age and gender divisions for groups.

LIABILITY RELEASE: We the undersigned do by our signing, release The Golf Institute and The North Texas Golf Center, their agents and owners from all liability, claims or other forms of legal action, from any damage and or personal injury resulting from any accident which may occur while the junior is participating in, traveling to and from, and or attending said league. I also authorize any medical and or hospital care that might be necessary for the junior. I agree to observe the USGA rules of Etiquette of golf, to conduct myself while at golf league properly, courteously, and to follow the facilities rules. I understand that I may be asked to not participate in the league for willful disregard of the rules, golf etiquette, or for dishonest, unsportsmanlike, or unbecoming conduct.
